Your appointment is now booked. We look forward to seeing you at our clinic. Please look for a confirmation email that should be sent shortly. You may need to check your spam folder.

When prompted by text or email, please confirm your attendance and complete check-in using the link in that text or email at least 24 hours before your appointment. The link will also provide directions to the clinic.

Please also plan to arrive 15 minutes early and bring the following items with you:

• Photo ID
• Current Insurance Card
• A list of all prescriptions you are currently taking
• A valid means of payment (i.e. debit or credit)

Copays, deductibles, and outstanding balances will be collected prior to your appointment.
